Welcome to MelMedic. These Terms of Service ("Terms") govern your use of the website melmedic.com (the "Site"), operated by MelMedic. By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

1. The Site is informational, not medical care

The Site provides general information about MelMedic's telehealth services and health-education content. Content on the Site is not medical advice, diagnosis, or treatment, and using the Site does not create a provider-patient relationship. A provider-patient relationship is established only through a scheduled visit conducted via our patient portal and is governed by the Telehealth Consent you review there.

MelMedic is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

2. Services and eligibility

Telehealth visits are available to patients located in states where Melanie Miller, MSN, APRN, FNP-BC is licensed: Arizona, California, Colorado, Delaware, Florida, Hawaii, Iowa, Kansas, Massachusetts, New Jersey, New Mexico, New York, Ohio, Oregon, Texas, and Washington. She holds full practice authority in Arizona, Colorado, Delaware, Hawaii, Iowa, Kansas, Massachusetts, New Mexico, New York, Oregon, and Washington. In California, Florida, New Jersey, Ohio, and Texas, state law requires a nurse practitioner to work with a collaborating or supervising physician, and care is delivered under that arrangement. Some services and prescriptions may be limited by state law. MelMedic provides care only to patients located in these states. Services are offered at the membership, program, and per-visit prices listed at booking (current fees are published on the pricing page and shown to you before you confirm a booking) and may change. Message membership covers messaging with your provider and does not include visits, medications, or laboratory testing. It is not insurance. MelMedic does not prescribe controlled substances or medical marijuana. All clinical decisions, including whether any medication is appropriate, are made solely by the treating provider. No specific treatment, prescription, or outcome is promised or guaranteed.

3. Booking, payment, and third-party services

Appointment scheduling, intake, records, and payment for visits are handled through a third-party HIPAA-compliant patient portal, subject to its own terms. Laboratory services are provided by third parties. Prescriptions are transmitted electronically to the pharmacy you choose. MelMedic is not responsible for the acts or omissions of third-party services.

4. Fees, refunds, cancellation, and no-show

Visit fees, including the $35 Refill Review appointment, are non-refundable once the visit has taken place.

You may cancel or reschedule up to 24 hours before your appointment at no charge. Cancellations made less than 24 hours in advance, and missed appointments, are charged a $20 fee to the card on file.

Message membership is billed monthly and may be cancelled at any time from your account.

MelMedic is a self-pay practice and does not bill insurance. Medications, laboratory testing, and imaging are billed separately by your pharmacy or laboratory.

5. Acceptable use

  • Do not use the Site to transmit health information; use the patient portal instead.
  • Do not misuse, disrupt, or attempt to gain unauthorized access to the Site.
  • Do not copy, scrape, or republish Site content except as permitted below.

6. Intellectual property

The Site and its content (text, graphics, logos, and design) are owned by MelMedic or its licensors and protected by intellectual-property laws. You may view and share links to the content for personal, non-commercial purposes; all other use requires our written permission.

7. Disclaimers

The Site is provided "as is" and "as available," without warranties of any kind, express or implied, including fitness for a particular purpose and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components.

8. Limitation of liability

To the fullest extent permitted by law, MelMedic and its owner will not be liable for indirect, incidental, special, consequential, or punitive damages arising out of your use of the Site. This section does not limit any liability that cannot be limited under applicable law, and it does not apply to clinical care, which is governed by the consents and agreements made within the patient portal.

9. Governing law and disputes

These Terms are governed by the laws of the State of Kansas, without regard to conflict-of-law rules. Disputes will be resolved in the state or federal courts located in Johnson County, Kansas.

10. Changes to these Terms

We may update these Terms from time to time. Continued use of the Site after changes are posted constitutes acceptance of the revised Terms.
