Welcome to MelMedic. These Terms of Service ("Terms") govern your use of the website melmedic.com [PENDING: confirm domain] (the "Site"), operated by MelMedic [PENDING: legal entity name and state of formation]. By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

1. The Site is informational, not medical care

The Site provides general information about MelMedic's telehealth services and health-education content. Content on the Site is not medical advice, diagnosis, or treatment, and using the Site does not create a provider, patient relationship. A provider, patient relationship is established only through a scheduled visit conducted via our patient portal (CharmHealth) and is governed by the Telehealth Consent you review there.

MelMedic is not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

2. Services and eligibility

Telehealth visits are available to patients located in states where Melanie Miller, FNP holds an active license with practice authority [PENDING-STATES: list of states]. Services are offered at the membership, program, and per-visit prices listed at booking ([PENDING-PRICE], confirm final pricing model) and may change. Membership visit allowances are subject to clinical appropriateness and scheduling availability, do not roll over, and are not insurance. MelMedic does not prescribe controlled substances or medical marijuana. All clinical decisions, including whether any medication is appropriate, are made solely by the treating provider. No specific treatment, prescription, or outcome is promised or guaranteed.

3. Booking, payment, and third-party services

Appointment scheduling, intake, records, and payment for visits are handled through CharmHealth, a third-party HIPAA-compliant platform, subject to its own terms. Lab services are provided by third parties such as Quest Diagnostics. Prescription fulfillment may be handled by your chosen pharmacy or by a licensed U.S. compounding pharmacy that ships under your prescription. MelMedic is not responsible for the acts or omissions of third-party services. [REVIEW WITH COUNSEL] Confirm refund/cancellation/no-show policy language. [PENDING: cancellation & refund policy details.]
