2. Services and eligibility

Telehealth visits are available to patients located in states where Melanie Miller, FNP holds an active license with practice authority [PENDING-STATES: list of states]. Membership visit allowances are subject to clinical appropriateness and scheduling availability, do not roll over, and are not insurance. 3. Booking, payment, and third-party services

Appointment scheduling, intake, records, and payment for visits are handled through CharmHealth, a third-party HIPAA-compliant platform, subject to its own terms. Lab services are provided by third parties such as Quest Diagnostics. Prescription fulfillment may be handled by your chosen pharmacy or by a licensed U.S. compounding pharmacy that ships under your prescription. 4. Acceptable use

• Do not use the Site to transmit health information; use the patient portal instead.

The Site and its content (text, graphics, logos, and design) are owned by MelMedic or its licensors and protected by intellectual-property laws. You may view and share links to the content for personal, non-commercial purposes; all other use requires our written permission.
